Palliative Care Nurse Practitioner – In Home Patient Visits

The Largest risk based, In Home Medical group with operations in markets across the US. Currently seeking In Home Nurse Practitioners to join their dynamic team of Clinicians for the Southern San Francisco area!

Palliative Care Nurse Practitioner – In Home Patient Visits

This organization is not a fee for service practice.

DESCRIPTION:

As the In Home Nurse Practitioner you are responsible for managing a small panel of patients with multiple, complex conditions working together with a nurse care manager (RN) and Primary Care Provider.  

Responsibilities include but, not limited to:

  • Perform comprehensive initial assessments for newly enrolled patients
  • Perform comprehensive initial assessments for newly enrolled patients.
  • Manage patient care on a longitudinal basis, deliver proactive care in order to reduce unnecessary hospitalizations.
  • Diagnose, prescribe and treat patients, consistent with standard of care.
  • Partner with the patient’s primary care provider and specialists to deliver integrated care.
  • Team based clinical environment, working with clinicians who are passionate about improving the system of care.
  • Participate in a rotated on call program for home based patient support after hours.
  • Accurate and efficient charting is essential to this role.
  • Participates in team meetings that include, but may not be limited to:
    • Weekly IDT meetings
    • Monthly staff meetings

 

Palliative Care Nurse Practitioner – In Home Patient Visits

SALARY/COMPENSATION:

  • Very competitive compensation and benefit package including; bonus program, marketing leading paid time off, CME reimbursement , auto allowance, child care benefits and more!
  • $140,000 - $155,000 with 20% bonus on top of salary base
  • Depending on experience!
  • 401k match after 90 days
  • CME reimbursement of $2,500 annually ( 3 days of paid leave)
  • Mileage reimbursement based on current IRS rate

 

QUALIFICATIONS:

  • Completion of Nurse Practitioner (NP) or Physician Assistant (PA – C) accredited program
  • 2+ years of clinical HANDS ON NP or PA experience preferred
  • Knowledge of clinical standards of care
  • Knowledge of long-term care, community resources and cost – effective alternatives regarding patient care delivery systems
  • House Calls will be required
  • Knowledge of CPT, ICD – 9 and HCPC Codes
  • Knowledge of Clinical Standards of Care
  • Awareness about UM Standards NCQA Requirements, CMS guidelines, Milliman guidelines, and Medicaid/Medicare contracts and benefits system would be preferred
  • Clinical training experience preferred
  • Current unrestricted NP or PA State license
  • Current State driver’s license
  • Passion for delivering care to patients with complex, chronic illnesses
  • Interest in conducting house calls.
  • Combination of geriatrics, palliative care or hospice experience as an NP or RN.

SCHEDULE:  Full Time, permanent position

  • Monday-Friday schedule visiting 5-6 patients per day
  • On Call:
    • On call is 1x every 6 – 8 weeks but, very limited hours and No Weekend ON Call
    • You will be compensated additionally for any On Call
  • Not necessary to have Palliative Care Nurse Practitioner experience
