Job Description

The Nurse Practitioner (NP):

• Gathers history and physical exam and diagnostics, as needed
• Develops and implements treatment plans given the patient’s goals of care and current conditions
• Meets the patient’s and family’s physical and psychosocial needs with support and input from the Interdisciplinary team
• Educates patients and families about medication usage, side effects, illness progression, diet nutrition, medical adherence and crisis anticipation prevention
• Contacts with other Clinical team members, patients’ other Physicians, and patients’ other medical providers to coordinate optimal care and resources for the patient and their family
• Maintains patient medical records and medical documentation consistent with state regulations and standards
• Participates in continuing education as required by the state Board of Nursing
• Prescribes medication as permitted by the state Board of Nursing
• Handles other duties, as needed

Required Skills

• 2+ years of Nursing experience
• Master's Degree in Nursing
• CA Nurse Practitioner (NP) license
• DEA registration or eligibility 
• CPR certification
• Solid assessment, clinical, and documentation skills
• Patient oriented
• Great interpersonal skills
• Excellent communication skills (written and verbal)
• Strong attention to detail
• Highly organized

• Previous experience in Home-based Services, Complex Cases, and/or Palliative Care
