We are currently seeking a Palliative Care Nurse Practitioner to join our team in Las Vegas. 

In this position you will primarily work with patients at one of the Optum Care Cancer Care centers to manage symptoms of their disease and the associated treatments, engage patients and families in goals of care conversations, and educate patients and families for improved understanding of their disease and the risks and benefits of treatments. This work will be coordinated with the Palliative Care team that follows these patients in their homes and when they are admitted to the acute setting. Under the direction of the Hospice Medical Director, you will assist in providing comprehensive medical care and provide direct care to cancer patients primarily in one of the Optum Care Cancer Care centers. There will be times when you will see Palliative Care patients – both with and without cancer – in their homes or other settings.

Primary Responsibilities:

  • Provide palliative care to cancer patients on site in one of four Optum Care Cancer Care centers
  • Attend weekly Palliative Care IDT meetings

Additional Responsibilities:

  • Visit Palliative Care patients at home, hospital or skilled nursing facilities as needed and as requested by supervisor
  • Assist with Palliative Care consults when needed
  • Rapidly formulate a differential diagnosis from assessments conducted within the clinical setting
  • Proficiency in developing and implementing treatment plans for patients assigned in the clinical setting, and documentation in the medical record
  • Provide guidance to nursing personnel towards treatment plans and care issues
  • Document collaborative guidance from physicians or other providers as needed
  • Coordinate care with the patient and family/caregivers
  • Basic technical proficiency in the use of computer programs for medical records or internet search for clinical practice
  • Ability to communicate with patients, families, health care team members and management staff about advanced illness, symptom management and goals of care
  • Ability to make care decisions based on best medical practices in specialty area of practice without direct supervision or guidance by a physician
  • Ability to identify areas for improvement for self and team, and to develop and implement plans to achieve them
  • Ability to work independently
  • Perform face to face evaluations of patients being evaluated for Hospice care

Required Qualifications:

  • Master of Science degree in Nursing 
  • Active, unrestricted APRN license in Nevada or ability to obtain
  • DEA license or ability to obtain
  • National Nurse Practitioner certification in Palliative Care, Family Medicine, Adult Medicine or Gerontology or ability to obtain 

Preferred Qualifications:

  • 1+ years of experience as an NP, preferably with Palliative Care, Oncology or Pain Management
  • Knowledge of end of life care, including advanced care planning, pain and symptom control of the terminally ill and bereavement care issues are desired
